Create Custom Consent Forms

1. Go to the 'Consent Forms' section of Rally and click on 'Add Consent Form'.

2. Enter the name and title of the consent form, along with the terms that participants must agree to in order to participate in studies.

3. Include a list of required statements that participants need to agree to by checking a box. Once completed, click 'Save Consent Form'.

Every time a participant signs a consent form, the form is stored in their Participant Profile

By following these steps, you can create custom consent forms in Rally to ensure that participants understand and agree to the necessary terms for their involvement in studies.
